Conventions: [mh] = MeSH heading
[majr] = MeSH heading designated as major topic
[pt] = Publication Type (PubMed)
[sb] = Subset of PubMed database (PreMEDLINE, Systematic, OldMEDLINE)
[sh] = MeSH subheading (qualifiers used in conjunction with MeSH headings)
[tiab] = keyword in title or abstract
[tw] = Text word
/de = controlled vocabulary heading in Dialog syntax (MeSH, Emtree, PsycINFO)
